We provide supported living that helps people live as independently as possible, in their own home or accommodation, with the right level of support around them. The focus is on choice, routine, safety, and building skills over time, not doing everything for the person. Our service is Nurse-Led and we accept private or self-referrals, NHS and Local Authority referrals.

What supported living looks like

Support is tailored to the individual and can include:

Daily living support

• Personal care (where needed)
• Meal planning, shopping, and healthy eating
• Support with household tasks and keeping the home safe
• Prompting and support with routines, appointments, and community access

Medication support

• Prompting, administering, and monitoring (based on assessment)
• Liaison with GPs, pharmacies, and community teams
• Clear records and safe storage processes

Community and wellbeing

• Support to access education, work, volunteering, and hobbies
• Building confidence using public transport, managing money, and social skills
• Support to maintain family links and positive relationships

Behaviour support and risk management (where required)

• Support based on a Positive Behaviour Support (PBS) approach
• De-escalation, proactive strategies, and consistent routines
• Risk assessments with clear guidance to reduce restrictive practice

Health appointments and multi-agency working

• Liaison with social workers, OTs, CMHT, district nurses and other professionals
• Support with hospital discharge planning and follow-up appointments
• Support with complex physical health needs and physical health

Who supported living is for

Supported living is suitable for various individuals. i.e. Adults who may be fairly independent but benefit from a structured, supportive environment. This includes people who are semi-independent and need ongoing support linked to learning disabilities, autism, mental health needs, physical disabilities, or long-term health conditions. It offers the chance to live in your own home with the right level of support, and you can personalise your space because it's your home.

• Promoting independence: We focus on "doing with", not "doing for", to help people build skills and confidence.
• Stable, consistent staff: We aim for continuity, clear routines, and respectful relationships.
• Quality and safety: Regular reviews, spot checks, supervision, and clear records to keep standards high.
• Flexible support: From a few hours 1:1 support a week to 24/7 support, including waking nights where needed.
